These methods range from collapsing the veins by sclerotherapy to physically removing the problem veins.
When dealing with large veins, endovenous laser ablation therapy is utilized. This technique has supplanted the need for surgery in the majority of cases today. This procedure involves the insertion of a very small laser fiber into the vein under sonographic guidance. Once inside, the laser fiber sends out energy that shrinks and seals the vein wall. Healthy veins around the closed vein restore the normal flow of blood. As this happens, symptoms from the varicose vein improve. Veins on the surface of the skin that are connected to the treated varicose vein typically shrink as well.
When there are remaining veins present after endovenous laser ablation, microphlebectomy is utilized. Microphlebectomy is a minimally invasive procedure that allows for the removal of large, superficial varicose veins through very small incisions that often do not require any stitches.
Small spider veins are treated using sclerotherapy. This treatment modality uses an FDA approved sclerosant, Sotradechol. The Sotradechol solution damages the vein allowing the body to reabsorb the vein and achieve the desired cosmetic result.
spider veins that cannot be treated using sclerotherapy can be treated using laser light energy. The laser energy passes through the skin to the level of the vein. The tissues in the vein absorb the light energy and cause destruction of the vein. This is used primarily for treatment of very small veins of the lower extremity and of the face. Most desired results for spider veins are achieved through a combination of sclerotherapy followed by laser treatments.
